1) Attach two passport size photos showing a very good likeness of yourself to the application form, together with a
     COPY [DO NOT SEND ORIGINALS] of the photo page of your passport.

2) Enclose a cheque for £108.00 payable to Colchester English Study Centre or, if you prefer, send credit card
     details. We do not accept any applications over the telephone.

3) Finally, send the completed form, full payment, a copy of the photo page of your passport, and the two passport-
     sized photos to:
